Consultation $50 (Consultation with practitioner to discuss treatment options. No treatment)
Initial visit $120 (detailed patient history + acupuncture)
Follow up visits $70 (acupuncture)

Naturopathy
Consultation $50 (Consultation with practitioner to discuss treatment options. No treatment)
Initial visit $180 (detailed patient history +Naturopathy)
Follow-up Visit $90 (Naturopathy)

First Line Therapy (FLT)
Consultation $50 (Consultation with practitioner to discuss treatment options.  No treatment)
Initial visit $145 (detailed patient history + FLT)
Follow-up visit $80 (Naturopathy + Acupuncture)

Payment: Due upon arrival.

Method of payment accepted:  Cash, Check, Master Card, Visa, American Express.

Insurance: The Samueli Center clinic has begun accepting insurance coverage for certain services.  We are accepting insurance coverage for services provided by the medical doctor and for some acupuncture, naturopathy and physical therapy services provided at the Clinic. It is important that you review your insurance plan benefits carefully to determine if your plan pays for the service(s) that will be received at the Clinic. Some insurance plans may require pre-treatment authorizations for certain services. There may also be a limit to the number of visits approved by your insurance for a particular service. If this is the case, we recommend that you obtain prior approval from your insurance carrier and provide a copy of the authorization for treatment to the Samueli Center clinic staff. If the plan does not cover a specific service you are expected to pay for the service provided.

CANCELLATION POLICY
